What sizes can Indian forge shops make?

From small parts under a kilogram to heavy forgings of several hundred kilograms, depending on press or hammer capacity.

Can I get flanges to ASME B16.5 or EN 1092-1?

Yes. Many suppliers forge and machine flanges to ASME B16.5 and EN 1092-1 with 3.1 certificates.

Do Indian forgings come heat-treated?

Yes. Most forge shops normalise, or harden and temper, in-house to the hardness you specify.
